He  should avoid all macrolide antibiotics(erythromycin, clarithromycin,
spyramycin).
Azithromycin is an erythromicin derivate and others macrolide antibiotics
are erythromicin derivates ,too.  Crossreaction is posible. But, you must
be sure to cause of allergy. Because
it is  possible that the allergy is to  azithromycin or to any adjuvante
supstance in suspension or cough syrup or new food (todlers). Allergy to
azithromycin  is long lasting ( long t1/2 of drug) versus allergy to other
supstance in suspension.
