We currently have a "NurseCheck" program at the hospital where I serve as the perinatal educator. It is an outpatient type service - no charge to the client or insurance companies - that includes assessment of mom and babe at approximately 3 - 5 days postpartum, if their docs won't be seeing them during that period of time. We have the BiliChek meter on trial right now. For the past several months, it has been apparent that the BiliChek will usually be off by a couple of points. It is recommended that, if the BiliChek is to be used, a baseline should be taken of all babies in the nursery before discharge so you know where you are starting at. Our docs do not wish to do this. In the past few weeks, we haven't been using the BiliChek, as the babies have to be pretty cooperative, moms get upset if we hold the babies' heads to get the four or five checks needed for an average, and if the kids come in glowing, we are going to call the physician and probably get an order to send the kid to lab anyway for a serum. I initially thought it was a great idea, and baby friendly, but it doesn't seem to do the job we thought it would. P.
