Who does take medicaid in your community?  When I opened a practice in
S. CA I had to limit medicaid to only those people in my zip code.
Other wise I would have been swamped.  Does this "boutique" practice you
are talking about charge much higher rates, or is there a large
"application fee" or other unusually lucerative compensation for these
physicians?  I think the idea of controlling who you work for and with
is great.  I encourage people to have free prenatal visits as an
opportunity to see if personalities and values fit.  The word
"exclusive" in common usage means limited to upper class people.  Simply
practicing in an "exclusive" area sets the tone of a practice and its
income.  To exclude certain people from your practice is not the same
thing as to be exclusive.  You will have to tell me more before I would
be willing to apply the lable "boutique" to the practice you describe.
Also I think practical economic considerations limit "boutique"
practices to wealthy metropolitan areas.  Are you practicing in a large
metropolitin area?
