The stories were similar in both cases...two-three day illness with high fever and decreased po intake. The nephrologists reminded all of us that since renal artery dilation is prostaglandin dependent during volume depletion, inhibition of prostaglandin synthesis turned off renal blood flow altogether. Who's likely to be volume depleted? A kid with a high fever, a sore throat, a gastroenteritis, etc, etc. We really discourage use of this agent for fever, although it is great to have it for pain and inflammatory disorders.
