Parents only see their kids at home, and really don't know waht happens at school except for what they hear from the child and the teacher. For that reason, we request evaluations from parents and teachers before even seeing a child for possible ADHD. We like to see the kids every 6 months while on medication, and before each visit we ask the parents to pick up enough forms for each teacher to fill one out. I find these invaluable in my evaluation of a patient with ADHD, and hesitate to change doses without them. Before we did this, we only had the parents' history to go on, and this is usually sketchy at best. The major problem we have now is getting the teachers to fill out the forms before the child comes in for the office visit. BTW, the forms we use are the ADDES.
