Distinguishing the advice service from your regular practice
may be a way to go with this. In our city we have at least one
hospital that operates a "warm line"  with basic child health advice
about feeding, fevers, behaviour, etc.  But, its very clearly a
telephone information service/not to be confused with a doctor
encounter.
