I've seen acute appendicitis present in a 10 year-old girl as anterior left thigh pain! It took several days (after an ortho consult and a bone scan) for the pain to finally land in the abdomen and the diagnosis of ruptured appy made. (Yes, I was convinced that the thigh pain, which was associated with vomiting, was the beginning of her acute appy and not an unrelated problem. Right now I can't remember all the details.) Of all the challenging cases I have seen, appendicitis has been the underlying diagnosis in a plurality (not, of course, in a majority). I have great respect for the many ways in which appendicitis can present in children.
