I had a mom once with something that presented just exactly like this only it
was bilateral.  I suspected it was eczema, related to her baby having just
started solids and still nursing, and she having a reaction of some sort to
his saliva.  I referred her on to a breast specialist at a breast clinic at
one of the hospitals in our area who concurred with my hunch,  but also
mentioned it is always good to get something like this checked out as it
could be Pagets' Disease.  The major difference in symptomatology, of course,
would be that eczema would present (probably) on both nipples, and Pagets'
(probably) only on one.
